MEDICAL INSURANCE:

HMO - Referral system that requires employees in New England to select a primary care physician (PCP).

PPO - Allows employees to select providers from either inside the network or outside the network at additional costs.

DENTAL INSURANCE:
Delta Dental
Requires selection of a dentist from the plan's participating group. Subject to deductible and annual maximum. Coverage varies across preventative, restorative, and major services categories. Date of hire or open enrollment

LIFE INSURANCE/AD&D:
Reliance Standard
Two times Base Annual Earnings (BAE) up to $325,000. Additional two times BAE up to $325,000 if a death occurs as a result of an accident. Date of hire

DOMESTIC PARTNER: Provides health and other benefits for eligible domestic partners of employees. Date of hire or open enrollment
SHORT TERM DISABILITY: Plan pays up to 80 percent of Base Monthly Earnings (taxable benefit) to a maximum of $2,000 per week for up to 90 days. Date of hire

LONG TERM DISABILITY:

Coverage begins after 90 days of disability and pays 60 percent of Base Monthly Earnings untaxed up to a maximum benefit of $8,000 per month. Date of hire

Employee is taxed on premium cost so benefit, if needed, is non-taxable.
401(k) RETIREMENT SAVINGS PLAN:
Fidelity Investments
Retirement savings plan with choice of 29 funds subject to maximum percent of total compensation and/or IRS annual limits. For 2008, election deferral maximum is $16,000. Plan accepts rollovers from previous employers. The IRS also allows "catch up" contributions for those age 50 or over. First of quarter following date of hire
FLEXIBLE BENEFIT:

Allows employees to set aside a portion of their income, pre-tax, to pay for expenses incurred for insurance premiums, un-reimbursed medical or dental costs, and dependent-care costs. IRS limits apply. Date of hire

NEW EMPLOYEE REFERRAL AWARD: Employees are eligible for a monetary award for referring a qualified candidate who is subsequently hired by EXFO. Date of hire
